As soon as you use the basic attack with the left mouse button, apply the skill «Bone Crushing» (R) (if you click more times, you can activate other unnecessary skills).
Then quickly type and use «Annihilation» (T) after «Bone Crushing» (R), an endless track of RTRT... etc.
It is not necessary to overload the skills of «Milling» (R) or «Breaking» (T), this may disrupt the sequence of the cycle.
